To create a new form or quiz, go to forms.google.com and log in (when prompted) if you are using a computer.
In case you are using an Android phone or iPhone, open a mobile web browser like Chrome and go to forms.google.com.

You can select from among the templates in the template gallery, by starting a new form from the beginning or duplicating a form you already made in the past.
